A company makes a mistake, but also makes the necessary correction steps ASAP. Who should resign/be fired?


I read many times recently that X, Inc. did a mistake in an area, but immediately took countermeasures. Then, after a few days (or during the actual incident) the responsible C-suite gets fired or resigns, and/or the person (or the whole team) who erred gets fired, too. Is that really necessary?
